If you shine a really bright light (in this case a 20mW laser) into a healthy tooth, it will illuminate the tooth evenly. In this case, you can see a sharp discontinuity on the far side. (It was easier to see in person; getting even a mediocre photo was tricky.) My dentist verified the tooth was cracked.
